Audit a supplier or a logistics partner and get a score you can defend. Most audit checklists give you a list of questions and a pile of opinions. This workbook gives you a score that shows how much of it you actually proved.
It covers suppliers and 3PL, 4PL and 5PL partners, including a section on the AI systems they use. You choose the scope, answer 140 questions across 16 areas, and the workbook calculates the result, ranks the gaps and prepares the report.
The 16 areas are: contract and SLA, quality system, qualification and regulatory standing, deviations and CAPA, receiving and storage, temperature control and cold chain, transport, inventory and traceability, security and falsified products, system connections and interfaces, data integrity, people and outsourced services, continuity and risk, 4PL orchestration, 5PL network and platform, and artificial intelligence systems.
What you get inside the workbook:
Every question has written level descriptions for scores 1, 3 and 5, so two auditors score the same situation the same way.
Evidence-capped scoring. An answer you only heard about cannot score above 3, and an answer with no evidence cannot score above 2. The dashboard shows raw and evidence-adjusted scores side by side, so you see how much of the result is proven.
A scope selector. Choose Supplier, 3PL, 4PL, 5PL or All, and questions that do not apply drop out of the scoring. Three yes or no questions suggest the right scope when you are not sure what the partner really does.
A dashboard with overall status, area scores, a radar chart against your target and the mix of evidence.
A Top 10 gaps list, ranked automatically.
A Findings and CAPA register with severity, root cause, corrective and preventive action, owner, due date and an effectiveness check. A finding cannot count as closed until the check is verified.
A one-page audit report, ready to print and hand over to the auditee or to your management.
A worked example with fictional data, so you can see how everything behaves before you start your own audit.
A Settings sheet where you can change weights, evidence caps, thresholds and target score.
Why 3PL, 4PL and 5PL. These labels are used loosely, and owning assets does not tell them apart. The workbook asks what the partner actually does: runs the operation, manages other providers, or runs a network or platform. It then adds the questions that matter for that role, such as provider selection and audit rights, conflicts of interest, cost integrity, exit, decision rights, data use by vendors and platform dependency.
The AI systems area has twelve questions on the register of AI systems, disclosure to the client, regulatory screening, accountable owner and human oversight, data governance, validation and drift monitoring, decision logs, change control, third-party AI vendors, incidents and fallback, security and the use of generative AI by staff. Common frameworks such as the EU AI Act, NIST AI RMF and ISO/IEC 42001 are used as orientation only.
Who it is for: quality, supply chain and procurement managers in regulated industries, consultants, internal and external auditors, and anyone who has to qualify or audit a supplier or logistics partner.
How it works: choose the scope, score each question from 1 to 5 and select the evidence behind it, read the dashboard and the top gaps, record findings and actions, then print the report.
The workbook works with formulas only, with no macros and no add-ins. Language: English.
Not included: this is a template, not consulting, certification or legal advice. Weights and level descriptions are the author's judgement and you can edit them. The questions describe good practice in general terms and reflect common GDP and GMP expectations; check the requirements that apply to you.
