This document is intended to provide a one-page, objective audit of new and existing factories as part of a companies supplier assessment process. It is geared towards manufacturing companies but could be modified to meet other types of suppliers and factories. Each criteria can be weighted according to the importance that is placed on it, and can vary depending on category and commodity. Scores are calculated based on entry of "x" in the appropriate box, and are tabulated at the bottom to give an overall score. Weighting and rating information are referenced at the bottom of the form. The form can be used either by an individual or preferably by a sourcing team to develop a baseline assessment that can be used along with other criteria to rate and assess a supplier (financial assessment, samples, etc).
