An ambulatory infusion center can grow revenue while increasing its exposure to drug purchases, delayed payer receipts and underused treatment capacity. This Excel financial and operating model connects those decisions in a 60-month forecast for independent infusion centers, specialty practices and their finance advisers. It supports planning for a new suite or a phased expansion using a structure of four therapies, three payers and three planned sites.
Begin by selecting the forecast start month and Downside, Base or Upside case. Replace the fictional sample assumptions with your own validated inputs. Drug assumptions include dose, vial contents, billing-unit conversion, acquisition costs, discounts, rebates, reimbursement bases, chair time, nursing workload and treatment frequency. Payer assumptions cover allowed-amount multiples, administration revenue, sourcing shares, denials, appeal recovery, patient collections, routine collection curves and paid waste. Site inputs specify opening months, chairs, RN staffing and setup capital.
The referral and authorization schedule converts expected referrals into approved starters and a continuing patient pool. Therapy mix and frequency produce requested visits. The capacity schedule independently tests chair hours and productive nursing hours, applies the tighter constraint and deducts unfilled cancellations. Completed visits feed 720 therapy-payer-month records, connecting treatment activity to contribution before payroll and fixed costs. Chair and nursing resources are pooled across active sites.
Buy-and-bill visits carry drug revenue and acquisition expense. Externally supplied drugs carry administration revenue and handling expense. Optional therapy-payer sourcing overrides support targeted comparisons. The economics view displays contribution by combination, contribution per visit, a sourcing comparison and weighted break-even visits and chair use.
Drug stock, payables and rebate receivables roll forward with purchasing, supplier terms and stock losses. Collections retain distinct timing for routine insurer cash, successful appeals and patient receipts. The funding schedule includes payroll, overhead, capital expenditure, depreciation, term debt, interest, tax losses, equity and revolving credit. Monthly financial statements reconcile to annual income, cash flow and balance-sheet summaries. A 13-week opening cash bridge allocates monthly flows using calendar-day overlap rather than individual settlement dates.
The executive dashboard combines six headline measures, four charts and formula-driven actions. A nine-point sensitivity grid tests Year 1 EBITDA against drug reimbursement and acquisition factors at active-case volume. Operating valuation discounts unlevered cash flows and uses explicit terminal replacement capital and working capital. Thirty-one checks review financial reconciliations and input consistency, with business warnings shown separately.
Editable formulas, input validation, internal navigation, instructions and source provenance support a transparent planning workflow. Teams can use the connected schedules to assess treatment economics, operational capacity and funding together, tracing key results back to the assumptions that drive them.
