Corrective and Preventive Action is required by almost every quality system, and it is also one of the areas where auditors most often find weaknesses: CAPAs that stop at "human error", actions without owners or dates, and CAPAs closed without any proof that the problem stayed away. The CAPA Toolkit turns the full CAPA discipline into a guided Excel workbook that a team can use from the first day.
The toolkit works through seven windows:
Window 1, Sectors: choose from 12 sectors, including apparel and textiles, automotive and engineering, electronics assembly, food and beverage, pharma and medical devices, hospital and healthcare, hotel and hospitality, banking and finance, IT and software, logistics and warehouse, construction and facilities, and education and public services. Each sector shows the standards it typically answers to, such as ISO 9001, IATF 16949, ISO 13485, FSSC 22000 and ITIL.
Window 2, Issues by SQDCM: the 10 most frequent issues for the chosen sector appear under Safety, Quality, Delivery, Cost and Morale. 120 ready issues in total.
Window 3, Root Cause: a Fishbone with six cause categories and a 5 Whys chain, pre-filled with the most common causes for the issue. The team checks the facts at the workplace, edits and confirms the root cause.
Window 4, Action Plan 3W: WHO (responsible department), WHAT (action) and WHEN (due date) for every containment, corrective and preventive action, with status flags for SET DATE, DUE SOON, OVERDUE and DONE.
Window 5, Problem Log: one line per finished CAPA. When the same issue appears again, the toolkit shows what was found and done before.
Window 6, Follow-up: KPIs for logged, open, overdue, closed, effective and recurred CAPAs, plus four charts (root-cause Pareto, CAPAs opened vs closed over 12 months, 3W action status, and issues by SQDCM) for a 15-minute weekly review.
Window 7, Self-Check: five automatic checks plus five self-ratings give your CAPA work a score out of 100, with Practitioner (70+) and Leader (85+) levels for an in-house review by the quality manager or CAPA sponsor.
The secondary document is a 23-page PDF guide book covering why CAPAs fail, each window step by step, the 120-issue library, a worked apparel DHU example, a 30-day rollout plan and an FAQ, plus a 10-prompt AI Prompt Pack for writing problem statements, checking 5 Whys chains and preparing the weekly review with any AI assistant.
The toolkit was developed from more than 20 years of Lean, quality and industrial engineering leadership in factories across Sri Lanka, China, Bangladesh, Indonesia, Thailand and Vietnam.
Requirements: Excel 2016 or later. No macros.
