Onboarding a supplier without a structured review exposes an organization to financial, legal, data, operational and reputational risk. This Vendor Due Diligence Scorecard gives procurement, compliance and third-party risk teams a consistent, documented and repeatable way to assess vendors, without the cost of dedicated third-party risk software. Every vendor is scored against the same criteria, every decision is supported by evidence, and every review date is tracked.
The assessment covers 40 due diligence questions across 8 weighted risk categories: Corporate & Legal, Financial Stability, Compliance & Sanctions, Information Security, Data Privacy, Business Continuity, Operational & Service Quality, and ESG & Reputation. Each question is scored from 0 to 5, or marked N/A, using a dropdown, and includes guidance on the evidence to obtain. N/A answers are excluded and the category weights rebalance automatically.
Six knock-out questions address the deal-breakers that matter most: unverified beneficial ownership, signs of insolvency, sanctions list matches, serious adverse media, refusal to sign a data processing agreement, and forced labour or human rights abuses. A failed knock-out question raises a red flag that overrides a good overall score, so a vendor with a deal-breaker is always escalated.
The workbook calculates an overall score out of 100 and assigns a final rating of Low, Moderate, High or Critical, each linked to a recommended decision: Approve, Approve with conditions, Escalate for senior approval, or Reject. The next review date is set automatically using the shorter of the rating cycle and the vendor criticality cycle, with Overdue and Due Soon alerts.
The template includes a Start Here guide, a Vendor Register for up to 50 vendors, the Assessment sheet, a 12-item Document Checklist with completion tracking, a one-page Scorecard per vendor with a sign-off block that prints cleanly or saves as PDF, a portfolio Dashboard with a criticality by rating heat map, and a Settings sheet to adjust weights, rating bands, review cycles and red-flag rules to match your own policy.
Two workbooks are included: a blank template ready for your vendors, and a sample workbook populated with six fictional vendors that demonstrates every rating outcome.
The template contains no macros or add-ins. Formula sheets are protected without a password to prevent accidental changes, and can be unprotected for customization.
This template supports, but does not replace, your organization's own professional judgement, policies and legal or regulatory obligations.
