Well-documented treatment plans are essential to facility-based care. Team input, patient involvement, regulatory oversight, and payer concerns all tend to hinge on good treatment planning. Stout and Jongsma developed a book and software (both published by Wiley) to aid with the idiosyncratic needs of treatment plans within continuum of care services (i.e., inpatient and partial hospitalization settings). Those wishing more detail as to data to use in constructing a treatment plan are encouraged to examine that work.
